Obicetrapib (Ubeslo, Evlarco): CETP Inhibitor Landscape

Obicetrapib, from NewAmsterdam Pharma and its European partner Menarini, is an oral cholesterol-lowering drug in the CETP inhibitor class. On September 21, 2026 it won the first regulatory approval anywhere in the world, in the European Union, and the class has a history of outcomes-trial failures. The open question for competitors is whether LDL lowering alone, or LDL lowering plus the coming PREVAIL outcomes trial, will carve out a place in a crowded market.

Mechanism at a glance

Obicetrapib inhibits cholesteryl ester transfer protein (CETP) and is taken as a 10 mg tablet, alone (EU brand Ubeslo) or fixed with 10 mg of ezetimibe (EU brand Evlarco). In the Phase 3 BROADWAY trial, patients with atherosclerotic cardiovascular disease or heterozygous familial hypercholesterolemia on maximally tolerated lipid-lowering therapy took 10 mg daily or placebo (2:1) for a year. LDL cholesterol fell 29.9% with obicetrapib and rose 2.7% with placebo at day 84, a between-group difference of 32.6 percentage points, reported in the New England Journal of Medicine (Europe PMC). The company’s topline release rounded this to 33%.

Why it matters

Four earlier CETP inhibitors had mixed fates. Torcetrapib was stopped early in a 15,067-patient trial after more deaths (hazard ratio 1.58) and cardiovascular events (1.25), with systolic blood pressure up 5.4 mm Hg (Europe PMC). Dalcetrapib was stopped for futility (hazard ratio 1.04) (Europe PMC). Evacetrapib showed no benefit (hazard ratio 1.01) and was stopped for lack of efficacy (Europe PMC). Anacetrapib in REVEAL cut major coronary events modestly (rate ratio 0.91; 10.8% vs 11.8%) over a median 4.1 years (Europe PMC). Obicetrapib’s EU authorization is its first approval anywhere.

The EU label rests on LDL lowering: the European Commission attached a condition that the company submit final PREVAIL results (NewAmsterdam 8-K). In TANDEM (407 patients), the fixed-dose combination lowered LDL-C 48.6% versus placebo, against 31.9% for obicetrapib alone and 20.7% for ezetimibe alone, as reported by the company in 2024 (8-K). Lipoprotein(a) is another thread; see our pelacarsen landscape for that market.

Competitive pressure points

  • Oral PCSK9: Merck’s enlicitide (Lipfendra), a once-daily 20 mg tablet, was approved by the FDA in July 2026; in CORALreef Lipids the adjusted LDL-C difference versus placebo was 55.8% at 24 weeks. Its outcomes trial is still ongoing (TCTMD).
  • Proven outcomes: evolocumab lowered first major cardiovascular events in VESALIUS-CV (3-point MACE hazard ratio 0.75) in people without a prior heart attack or stroke (Europe PMC); alirocumab had a hazard ratio of 0.85 in ODYSSEY OUTCOMES (Europe PMC).
  • Twice-yearly siRNA: Leqvio sales were $506 million in Q3 2026 (Novartis); see our inclisiran landscape.
  • Monthly PCSK9: lerodalcibep (Lerochol) was announced as FDA-approved on December 15, 2025 (TCTMD).

What to watch

  • PREVAIL: a placebo-controlled trial of obicetrapib added to maximally tolerated therapy in patients with atherosclerotic cardiovascular disease and/or HeFH. Enrollment of over 9,500 patients finished in April 2024, and the company’s first interim readout is a blinded data-monitoring committee decision, not full data.
  • The US path: management said at a September 2026 investor conference it plans to file and launch roughly 12 to 18 months after PREVAIL finishes.
  • Whether European payers reimburse an LDL-lowering label without outcomes data.

  • Sep 21, 2026: The European Commission authorized Ubeslo and Evlarco, described as the first approval of obicetrapib worldwide. • Sep 9, 2026: At the Wells Fargo healthcare conference, the company said it expects to reach the PREVAIL interim event count in the fourth quarter, with a readout in the first quarter of 2027. (Investing.com transcript)
  • Aug 5, 2026: Second-quarter report: cash of $678.3 million at June 30; RUBENS (about 300 patients) topline expected by year end 2026. (NewAmsterdam 8-K)
  • Jul 23, 2026: The EMA’s CHMP adopted positive opinions for both products. (EMA)
  • Jul 2025: BROADWAY was published in the New England Journal of Medicine (2,530 patients). (Europe PMC)

Bottom line

As of November 4, 2026, obicetrapib is approved in Europe on LDL-lowering data, with no US application that we could verify and an outcomes trial that will decide its broader role. The class history means the market will wait for PREVAIL before treating it as more than another LDL-lowering option.
