Semaglutide (Wegovy, Ozempic): Landscape and Generics

Semaglutide, Novo Nordisk’s GLP-1 medicine sold as Ozempic, Wegovy and Rybelsus (and, since January 2026, as a Wegovy pill), was the world’s best-selling medicine in 2025, with combined sales of $36.19 billion according to BioSpace. In 2026 the molecule’s patent has expired in several large markets, generics are on sale, and Eli Lilly’s tirzepatide is the main branded rival. This is the position as of November 4, 2026.

Mechanism at a glance

Semaglutide is a GLP-1 receptor agonist, given as a weekly injection or a daily tablet. US labels (DailyMed) cover several uses. Ozempic injection: glycemic control in adults with type 2 diabetes, reduction of major cardiovascular events in adults with established cardiovascular disease, and reduction of kidney outcomes in adults with type 2 diabetes and chronic kidney disease. Rybelsus and Ozempic tablets: glycemic control and major cardiovascular events in high-risk type 2 diabetes. Wegovy injection: major cardiovascular events in adults with established disease and obesity or overweight, weight reduction (adults, and patients aged 12 and older with obesity), and noncirrhotic MASH with moderate to advanced fibrosis under accelerated approval.

Why it matters

In SELECT (17,604 adults with cardiovascular disease and overweight or obesity, no diabetes), the primary event occurred in 6.5% on semaglutide 2.4 mg versus 8.0% on placebo (hazard ratio 0.80). In FLOW (3,533 people with type 2 diabetes and chronic kidney disease), the primary kidney and cardiovascular composite occurred in 331 versus 410 patients (hazard ratio 0.76). In ESSENCE, which supported the August 2025 accelerated approval for MASH, steatohepatitis resolved without worsening fibrosis in 62.9% on Wegovy versus 34.1% on placebo at week 72.

Competitive pressure points

  • Generics: see the dated items below. Novo cites “negative impacts from the compound patent expiry” in certain markets. BMO Capital Markets does not expect this to reach the US, where it calls Novo’s IP “iron clad” (BioSpace).
  • Tirzepatide (Lilly): in Q3 2026 Lilly reported Mounjaro revenue of $10.7 billion and Zepbound $5.35 billion. In SURMOUNT-5, weight loss was 20.2% versus 13.7% for semaglutide. See our comparison guide and the tirzepatide landscape.
  • Oral rivals: Lilly reported $184 million of Foundayo revenue in Q3 2026; see our orforglipron spotlight. Novo’s CEO said it holds about 80% US share of the Wegovy pill market.
  • Next-generation agents: Lilly reported that retatrutide beat Ozempic 2 mg in a Phase 3 type 2 diabetes trial (retatrutide spotlight). Novo’s own CagriSema is covered in our CagriSema guide.
  • US pricing: from January 1, 2027, Novo’s US list prices for Wegovy and Ozempic fall to $675, cuts of roughly 50% and 35%.

What to watch

  • How much generic entry erodes Novo’s international sales beyond Canada.
  • Whether China approves semaglutide copies; we found filings, not approvals.
  • US decisions on the Wegovy pill in MASH and on CagriSema.
  • A Fifth Circuit decision in the FDA shortage-listing litigation, which Novo expects before the end of 2026.

Upcoming catalysts

  • Q4 2026: Novo lists US decisions on oral semaglutide 25 mg and on CagriSema.
  • Jan 1, 2027: US list price cuts to $675 for Wegovy and Ozempic take effect.
  • Q1 2027: expected US decision on the Wegovy pill in MASH.

Bottom line

Semaglutide carries labels across diabetes, obesity, cardiovascular, kidney and liver disease, but exclusivity has ended in several markets and Novo is cutting US list prices from 2027. The competitive question is how fast generics and tirzepatide erode a franchise that was the world’s largest in 2025.
